Overcoming Addiction Since 1984

This treatment is designed for male and female Ontarians aged 16 and over with a long history of severe and persistent co-occurring addiction and mental health problems, including court-mandated individuals and clients usually refused admission to other programs because of psychotropic medication use. Typically, clients have multiple and complex clinical/social and other related needs which often remains unmet through programs of lesser intensity and structure, such as Day treatment.

This is an intensive, structured 28 day live-in treatment, recovery, housing and support in a therapeutic community environment, designed for (but not exclusively) hard-to-treat populations with a history of severe and persistent co-occurring addiction and mental health problems. Through a holistic approach, client progress is intensely monitored and services include individual & group counseling, educational sessions, interactive workshops, peer support, life skills training and discharge planning/preparation for reintegration to home and family, school/work/life, and/or referral to longer-term treatment and rehabilitation.

Admission

Our Process

The admissions process for treatment at Centre Jubilee is easy and convenient. First you can begin by completing our intake form below, you can send this by email, fax or mail. In order to be considered for admission to the residential program you will be required to complete an assessment using provincial standardized assessment. In the Cochrane district you may contact South Cochrane Addiction Services or North Cochrane Addiction Services to schedule your assessment. Elsewhere, please contact the Drug and Alcohol Registry of Treatment Agencies in Ontario at 1-800-565-8603 or contact a Ministry funded addiction services agency near you. 


You will also be required to have Medical forms fully completed by a physician, nurse or nurse practitioner and returned to us either prior to or immediately following the above step. Otherwise, the admission process will not proceed further. The completed medical form may be faxed, emailed, mailed or couriered.
Once all of your information has been submitted we will start by speaking to you over the phone.  Finally, we’ll provide you with a date for admission into our Drug Addiction Treatment Center where we’ll introduce you to our medical staff and caseworkers, create your individualized treatment plan and start your recovery process.
